Blue Cross and Blue Shield of Vermont and The Vermont Health Plan are required to notify you of certain legal rights and policy limitations that may apply to your coverage with us. This document contains the following important notices and information:

  • a notice of your rights under the Newborns’ and Mothers’ Health Protection Act, which requires that we provide certain minimum benefits related to hospital stays for childbirth;
  • a notice of your rights under the Women’s Health and Cancer Rights Act of 1998, which requires that we provide certain minimum benefits for reconstructive surgery related to mastectomy;
  • a notice of preexisting condition exclusions, which explains what a preexisting condition is and how your benefits may be limited if they relate to a preexisting condition;
  • a notice of special enrollment rights for group health plan members, which describes the circumstances under which you may be allowed to enroll in your employer’s group health plan or add dependants to your group health plan policy outside of the normal, open-enrollment period; and
  • our Notices of Privacy Practices, including a summary of those notices, which describe the various ways that we may use and disclose your confidential information in the course of providing you with health benefits.
