Small Business QHP Direct Enroll Tool
This tool is intended for small businesses* that had their 2013 plans extended for up to three months until March 31, 2014 or that have insurance plans that renew between January and April 30, 2014.
*Small businesses have at least one employee, who is not the owner or their spouse, and an average of 50 or fewer full-time employees that worked on working days during the preceding calendar year.
What you'll need
Gather the following information to complete the online enrollment form:
- BCBSVT Group Number or VACE ID & EIN
Employee information for employees currently enrolled in your health plan offering (dependent information is not required)
- Date of Birth
- Social Security Number (only for VACE)
- Plan Selection
Complete the Online Enrollment Form
If you are a VACE small business, use this form: BCBSVT VACE QHP direct enroll form
- All other small businesses should use this form: BCBSVT QHP direct enroll form
Note: Employees will transfer to thier current plan type (i.e. single or family). You will need to complete and submit a Small Group Change Form for any changes listed below:
- Add a new employee/hire
- Change plan type (i.e. single to family)
- Add/remove dependents
- Cancel existing coverage
