Mental Health/Substance Abuse Integrated Care (Vermont Collaborative Care, LLC)
Vermont Collaborative Care, LLC (VCC) will begin operations on July 1, 2013 providing integrated care management services to Blue Cross and Blue Shield of Vermont and The Vermont Health Plan members. We are working with Magellan to transition your relationship with Magellan directly to BCBSVT and most importantly transition care of our subscribers to ensure seamless and enhanced continuity of care.
