Important Reminder

Changes for Anniversary and Open Enrollment for Large Groups

  • You may submit membership changes twice a year, during your anniversary or during open enrollment if applicable.


  • If your anniversary is January and we receive your paperwork in December, the change will be effective January 1st. If we receive the paperwork in the month of January the effective date will be February 1st.


  • It is suggested you submit all paperwork the month prior to the requested effective date.
