Case Management

The specialty case management program is voluntary and available at no cost to members of Blue Cross and Blue Shield of Vermont and The Vermont Health Plan. You will receive personalized care-planning by one of our registered nurse case managers. Your case manager will work with you, your family, and your physician to coordinate the medical care most appropriate for you. Your case manager will work as an advocate, helping you in managing the benefits you receive through your health plan, as well as identifying other programs, services, and support systems available to you.

Q: What Services Will My Specialty Case Manager Provide?

A: Your specialty case manager will review your condition, provide a comprehensive assessment of your medical situation, work with your care providers to coordinate a plan of care that will optimize your health insurance benefits and assist in arranging needed services. Other services include:
  • Coordination of Services - Maintaining contact with your physicians and other care providers, arranging for medical services and equipment and coordinating your benefits.


  • Education - Providing educational material, training resources, and recommendations about community-bsed services.


  • Strategy - Monitoring incoming data, helping you avoid duplications in service, and finding ways to minimize your out-of-pocket expenses.


  • Problem Solving - Answering your questions ,recommending appropriate actions and responses and helping you troubleshoot problems as they arise.

Q: Who Is Selected for Case management?

A: In the case of a chronic illness or catastrophic health event, we're here to help by answering your questions and providing you with the information and resources you'll need. Our specialty case management program was developed to help you and your physician manage your care and appropriately utilize the benefits you receive from your health plan.

Q: Does This program Change My Health Benefits?

A: No. Your existing health benefits remain the same. You may continue to use the same care providers. You will incur no additional costs by enrolling in Specialty Case Management.
