Chronic Care Management Can Save You Money As part of our robust Blue HealthSolutions health management programs, we specially designed a program to help our members who wrestle with certain chronic conditions get the care they need. This Chronic Care Management program, which we tailor to meet individual needs, may include a variety of components, for example:
 | | | | | | | • | Special outreach, counseling and coaching from our disease management doctors and nurses; and/or
| | | • | Specialty case management on an ongoing basis or to help members with specific episodes.

Participation
Participation in one of our high-intensity Chronic Care Management programs may save you money. If you participate in a high-intensity program and comply with care recommended through that program, we will waive co-payments and other cost sharing for care you receive for your chronic condition. Please note that only certain individuals with a limited list of conditions will qualify for this benefit. Your eligibility for a program will depend on the nature and severity of your illness. Chronic care management is distinct from our other Blue HealthSolutions programs in that it involves active participation from the member and interaction with the member’s physicians and other providers.
