Getting Started - 1st Time Users

Option 1


1. Ask your doctor to write two prescriptions for your long-term medications:
  • One for a short-term supply (e.g. 30 days) to be filled immediately at a participating retail pharmacy
  • One for the maximum days' supply allowed (up to a 90-day supply) with as many as three refills (if appropriate) to be mailed to Caremark
2. Fill out the mail service order form
Option 2

Prescription by Phone - Provide your doctor's office the FastStart® toll-free physician number; 1-800-378-5697 and let your prescriber call in the prescription. To expedite processing, please provide your doctor with your ID number (from your ID card) and mailing address.


Refill/Renew Order one of three ways:

Online - Order your mail service refills by using Caremark.com. Register online to receive email reminders, informative newsletters and other e-mail alerts. Have your member ID number handy to register.

By Phone
- Call your toll-free Customer Care number, 888-543-5929 for fully automated refill service. Have your member ID number ready.

By Mail - Once you receive your medication, keep your refill labels to use on your next order form. Attach the refill label provided to the mail service order form and enclose your payment with your order, if your plan requires a payment.
