Select a carrier plan through the State of Vermont
(premium assistance granted)

Great, you've been approved for premium assistance.  Now you select the carrier plan through the State - Blue Cross Blue Shield of Vermont.
 

Payments: Catamount Health members who receive premium assistance pay the State of Vermont for their portion.  The State of Vermont pays the full premium to the carrier.

Complete a carrier plan application
(premium assistance denied)

If you were denied premium assistance, complete an application through the carrier - Blue Cross Blue Shield of Vermont .  Remember to enclose the first month's payment with your application.

 

Payments: Catamount Health members who are not eligible for premium assistance pay the carrier directly.

 

Differences between “Green Mountain Care” and “Catamount Health”

Green Mountain CareNew window  is the name of the State of Vermont’s health program. Catamount Health is just one of the plans in the Green Mountain Care collection. Others include Medicaid, the Vermont Health Access Plan and Dr. Dynasaur. You only need apply once to determine eligibility for all Green Mountain Care programs.
