Reading Your EOB (Explanation of Benefits)

 In response to customer suggestions, your Explanation of Benefits (EOB) is sporting a new look that’s easier to read and understand! Remember, your explanation of benefits is NOT A BILL, it is a statement that helps you understand how your benefits were paid. 

Familiarize yourselft with the new EOB format using our interactive mouse-over.  Simply place your curser over any of the highlighted fields and a brief description will appear.

Features of new EOB

Increased awareness of ‘Check Enclosed’– if a check is attached to your EOB:

• Phrase “CHECK ENCLOSED” will appear in red on front of EOB with check number, and
• Check will be last page of EOB

Identification number on right hand side – to increase the protection of your health information your identification number has been relocated from the left to right hand side of your EOB.

New claim details – the following claim information is now provided:

• Non-Covered Due From Patient
• Non-Covered Not Due From Patient
• Other Insurance Payment

Updated descriptions of EOB terms – the glossary of EOB terms will continue to be located on the last page of your EOB, unless a check in attached, in which case it will be the next to last page.
