Your Explanation of Benefits (EOB) is a statement that describes how your health benefits cover services received, including:

• what services we (the plan) pay
• what you (the member) pays
• what services we don't cover and why

For every claim filed, we generate an EOB.  The EOB is not a bill.  Your provider will send you a bill for any amounts owed.
