Distinct from other Programs

As a part of our robust Blue HealthSolutions (BHS) health management programs, we've designed a program to help our members who wrestle certain chronic care conditions get the care they need and save money.

The chronic care management program is distinct from our other BHS programs in that it requires active participation from members.  Members must comply with program requirements such as, periodic interaction with their physicians and other providers.

Tailored to Individual Needs

Eligible participants will receive a  program tailored to their unique needs, including:

• special outreach, counseling and coaching from our disease management doctors and nurses; and/or
• specialty case management on an ongoing basis or to help members during specific episodes.

Participation may save you money

Members actively enrolled in one of the below indicated chronic care management programs (CCMP) will have waiting periods for pre-existing conditions waived.  In addition, all patient cost-sharing (deductible, coinsurance, co-payments) associated with treatment of the chronic condition for which the member is enrolled in a CCMP will also be waived.

Qualification & Eligibility

Here are the chronic conditions for which chronic care management is available under the Catamount Blue Health plan:

• cardiac disease
  
  • congestive heart failure (CHF)
  • coronary artery disease (CAD)
• asthma
• diabetes
• chronic obstructive pulmonary disease
• anxiety disease
• bipolar disease
• major depression
• post-traumatic stress disorder
• schizophrenia
• substance abuse
